The medical situation on Roatan

The public sector

The dentistry and medical services on Roatan are clearly segregated into public and private sectors. Publicly funded hospitals often face severe resource shortages, to the extent that access to running water can sometimes become a problem. If possible, it is advisable to avoid seeking treatment at a public hospital. Individuals accustomed to first-world standards may find the conditions there to be quite shocking.

Having said that, I personally have experience with the public hospital system in Roatan. I underwent surgery there and also visited a public hospital daily for two months to receive consistent wound care. I am deeply appreciative of the dedication exhibited by the staff at the public hospital, despite their profound lack of essential equipment and medicines.

On one occasion, after a procedure, I was asked to locate and purchase a bottle of local anesthetic because they had exhausted their supply during my surgery. With an IV still in my wrist, I hastily hailed a taxi and scoured several pharmacies in search of this crucial vial. After five stops, I finally found a pharmacy with five available bottles, each priced at only 50 Lempira. I decided to purchase all of them. The nurses were incredibly grateful, and I felt as though they might throw a party in my honor. It was a truly humbling experience and provided me with a crash course in the disparities between the “haves” and the “have-nots.”

As a point of reference, the surgery I underwent involved a single surgeon suturing a 7-inch wound on my stomach, with two nurses assisting, and all sterile supplies were used. The total cost, including the aforementioned vial, amounted to 1,200 Lempira, which is equivalent to $50. The Honduran medical system can simultaneously evoke despair due to its lack of resources and admiration for the dedication and skill exhibited by its doctors.

The private sector

In cash-based clinics and hospitals, you can access advanced surgical techniques, private rooms with en-suites for your family, stem cell therapy, and essentially everything you would expect to find in a first-world country, sometimes even more. However, given the island’s size, it cannot accommodate specialists from every field of medicine on a permanent basis. Many specialists visit the island once a month for consultations. If you require procedures that are not available on the island, cities like La Ceiba, San Pedro Sula, and Tegucigalpa boast excellent hospitals.

I have a personal history closely intertwined with medical care in Honduras, ranging from the loss of my wife over a decade ago after a lengthy illness to a near-fatal scooter accident I experienced five years ago. Consequently, I have developed numerous opinions on the matter.

On the day of the scooter accident, I was unaware of the severity of my injuries, and I remained quite lucid. They initially attempted to transport me to the public hospital, but I insisted on being taken to Woods Clinic. It’s not an exaggeration to say that they saved my life that day. I had internal injuries, internal bleeding, multiple organs that required repair, and some intestines needed to be removed. Despite having 13 broken bones, my overall mood was surprisingly good. It wasn’t until I utilized my limited Spanish skills that I realized the doctors weren’t offering favorable odds for my survival. Without this insight, I might not have fully grasped the gravity of my injuries.

It’s worth noting that even though I had funds in the bank and comprehensive insurance, the surgeons refused to proceed with the operation until they had received sufficient cash to cover a portion of the procedure. While this might appear ruthless and unfeeling, it’s a response to the unfortunate reality that some expatriates on the island have previously utilized private hospital services and then evaded settling their bills.

The surgeries did eventually proceed, primarily due to a predatory lending group that, knowing I had insurance, convinced me to sign a document while I was alone in an emergency situation and under the influence of morphine. This document granted them the authority to advance money to the hospital and bill my insurance. I firmly believe that Woods Clinic and the dedicated surgeons received only a fraction of the cost for those operations that day, whereas my insurance was billed for tens of thousands of dollars.

I have much more to say on this subject, including my attempt to file a complaint. However, I will emphasize one more point: while having insured individuals on the island has led to the establishment of multiple ambulances and 24-hour clinics, many of these facilities bill at rates more in line with first-world standards than with the typical costs of Honduras. While this influx of medical facilities may seem advantageous, I am skeptical about whether it truly benefits those who cannot afford such care or lack insurance.

Diseases and issues common on Roatan

We reside on a tropical island where the climate is characterized by high temperatures and humidity, creating an ideal environment for the proliferation of bacteria, viruses, and fungi. Here are some fundamental tips. You can find a comprehensive list of recommended vaccinations on the CDC website.

t’s important to note that these recommendations apply to Honduras as a whole, and while I haven’t heard of any reported cases of rabies on Roatan or the mainland for years, it’s worth mentioning that the USA sometimes uses this as a reason to restrict the movement of pets between countries.

Malaria poses a concern, but in general, the side effects of prophylactic medication can be quite unpleasant. Given the rarity of contracting malaria in this area, I would advise against taking preventative medicine. The type of malaria most commonly found on the island is curable, and with medication, you can typically overcome the worst symptoms within two days. However, if you experience flu-like symptoms, it’s essential to get tested, as malaria can be a debilitating illness but can be treated quickly.

Dengue is a virus, and unfortunately, there’s no specific treatment for it. If you do contract dengue, the best you can do is stay hydrated, take pain relievers, and hopefully have a friend to support you during your recovery. In my 17 years here, I’ve experienced each of these illnesses once, which was more than enough.

Skin conditions are prevalent in this environment. It’s crucial to treat and clean every scratch or cut immediately. Those who are particularly susceptible to adverse reactions from sand flies and mosquitoes should resist the temptation to scratch these bites, as it can lead to infections.

If you notice changes in the color of patches of skin, patterns spreading, itching, or burning sensations, it’s advisable to consult a doctor. Most likely, it’s a manageable skin infection. We do have dermatologists visiting from the mainland regularly, although many local doctors are well-versed in addressing such issues.

Basic ailments what to do!

For minor illnesses and common health issues, I highly recommend Clinica Esperanza in Sandy Bay. Unfortunately, it’s only open from Monday to Friday, but for a nominal fee of 500 Lempira, you can receive prompt medical attention. They have a team of local Honduran doctors and even U.S. doctors who are in their final years of training and gaining experience here. Additionally, if they have the necessary medication available, you may receive it for free. The 500 Lempira fee goes toward supporting their provision of free medical services to the community. You can also check their Facebook page, as they often seek assistance with supplies or couriers to bring donated items from the USA.

Another option is AKR, which has a clinic with a decompression chamber for divers. While it may be more expensive, you’ll need to cover the cost of your medications, which is typically at regular pharmacy prices.

In Coxen Hole, right across from Petrosun, you’ll find Island Medical. This facility is excellent for conducting medical tests, and there’s usually a doctor available as well. I had a personal experience where a doctor in the USA recommended I undergo 17 different blood tests, which would have cost several thousand dollars at least. However, at the Island Medical clinic, I was able to get these tests done for approximately $300. Once again, the costs here align with standard Honduran practices, and there’s a pharmacy nearby in case you require any medications.

Emergencies

In the event of a severe emergency, circumstances can escalate rapidly, and you may not have the opportunity to make a choice. Typically, you will be transported to the public hospital unless someone intervenes on your behalf to have you taken to one of the private hospitals, such as Woods or Cemesa.

Can I get insurance on Roatan?

If you are a resident, you have the option to obtain Honduran insurance. You can reach out to GRACIA MARIA MADRID CASTRO at Phone: 2216-4700. (Please note that all landline numbers in Honduras consist of 8 digits and begin with a 2). She would be delighted to assist you. However, please be aware that all communication with her will be conducted in Spanish.

Additionally, there are representatives from other insurance companies on the island. Since these companies may not be based in Honduras, obtaining residency in Honduras may not be a requirement, although they may ask for residency in another country.

I’ve been using their services for 17 years, and I’m generally quite satisfied with them. When I first enrolled, my premium covered two adults in their 30s and three children, totaling around $2,500 per year. However, over the years, the premium has steadily increased. Now, as a single man in his 50s, I’m paying $6,000 annually just for my coverage. Despite the rising costs, I must say that they have provided me with excellent service. When I’ve been overseas, they’ve reimbursed almost all of my expenses once I met my deductible.

Their coverage also includes emergency evacuation to any part of the world, and they’ve paid out several hundred thousand dollars in medical bills on my behalf. While their insurance works in the USA, I personally prefer to seek medical treatment outside of the USA, where the quality of care is just as high, and the prices are not exorbitant.

Patients typically cover the cost of the doctor’s consultation separately from any required treatment or medications for their condition.

The staff is highly responsive on WhatsApp, although it’s important to mention that they primarily communicate in Spanish, while the doctors are proficient in English.

Roatan Hospital is situated in Coxen Hole and is a part of the Honduras Public Health Care System. As a result, this hospital does face several limitations. Despite efforts from various community initiatives to augment these shortcomings, the hospital still grapples with issues such as flooding, a shortage of running water, and inadequate supplies.

However, it’s worth noting that many of the surgeons and doctors who work at other hospitals also dedicate their time to the public hospital. Consequently, you might have the same doctor attending to you regardless of the hospital you visit. These healthcare professionals do remarkable work despite the limited resources available to them.

We are fortunate to have several highly skilled dentists on the island, equipped with the latest technology and services. Dental procedures in Honduras tend to be more affordable than in many other countries. In fact, I’ve known several North Americans who have traveled here specifically for dental work. Even when factoring in the cost of a vacation, they found that having the work done in Roatan was still more economical than getting it done in their home countries. While recovering on the beach might not be the typical idea of a vacation, it’s certainly preferable to paying higher costs and enduring discomfort at home.
